Medicare is a federal health insurance program designed for people 65 years of age or older and certain younger people with disabilities. It covers 100% of qualifying home health care costs. There are no deductibles or copays. Medicare also covers nursing, physical therapy, speech therapy and occupational therapy. A home health aide and/or medical social worker may be provided along with one of the above services. Medicare services are on an intermittent visit basis, not on an hourly basis, and are intended to be short term.
Medicare Home Health Benefit
To qualify for the Medicare Home Health Benefit, you must meet all of the following criteria:

You must be a Medicare beneficiary

Your doctor must decide that you need medical care at home, and make a plan for your care at home

You must be homebound, or normally unable to leave home. Leaving home takes considerable and taxing effort. A person may leave home for medical treatment or short, infrequent absences for non-medical reasons, such as a trip to the barber or to attend religious services.

A need for adult day care does not keep you from getting home health care for other medical condition.

You must need at least one of the following:

Intermittent (and not full time) skilled nursing care

Physical therapy or speech language pathology services

An appointment with your health care provider within 3 months before or 30 days after you start home care. related to the reason for home care.

Medicaid or Other Private Insurance
Medicaid follows the Medicare rules for the most part and there is no copay or deductible for home visits. Private insurers have their own requirements based on the specific insurance plan of the individual, so deductibles and copays may apply.
